Support Coordination

Our Support Coordination service helps NDIS participants and their families navigate the NDIS system with confidence. We assist individuals to understand their NDIS plans, connect with appropriate service providers, and coordinate supports that align with their goals and needs. Our team works collaboratively with participants, families, and providers to ensure supports are working well together, barriers are addressed early, and participants are empowered to build independence and make informed choices about their supports.
